When you book a New Patient appointment you will receive a confirmation email and digital versions of our intake forms.
Please take the time to fill in all forms at least 30 minutes PRIOR to your first visit to ALINIO.
Your practitioner needs time to review these forms ahead of your appointment to ensure that your first visit is a great success. Please note that incomplete intake forms may require us to re-schedule your first appointment to a later time.
