When you book a New Patient appointment you will receive a confirmation email and digital versions of our intake forms.
Please take the time to fill in all forms at least 30 minutes PRIOR to your first visit to Alinio.
Your practitioner needs time to review these forms ahead of your appointment to ensure that your first visit is a great success. Please note that incomplete intake forms may require us to re-schedule your first appointment to a later time.
If you have any questions, or would prefer to talk to an Alinio team member before booking your first appointment, please feel free to contact us by phone or email.
We have an in-office digital device for completing forms and hard-copy paper forms if required. Please Note: you will need to arrive 30 minutes BEFORE your appointment time if you wish to use either of these methods for submitting the intake forms.
